Payment for services rendered is expected at the conclusion of the office visit. To facilitate this, Dr. Prystowsky accepts most credit cards, checks, and cash. Dr. Prystowsky does not participate in any insurance plans, but at the time of payment, all patients receive a bill that may be submitted to their insurance carrier for reimbursement. The billing desk will attempt to assist patients with questions they have concerning submitting to their insurance carrier for reimbursement. If after submitting to the insurance carrier the office receives the reimbursement check, Dr. Prystowsky will send a refund to the patient for the amount of the reimbursement.

As of July 1, 2003, Dr. Prystowsky opted out of Medicare. This applies to all existing patients and any new patients. New and existing Medicare patients are welcome to see Dr. Prystowsky, but their claims will not be submitted to Medicare. Payment to Dr. Prystowsky is expected at the time of service and will be completely outside of the Medicare program. If you have additional private insurance that is not a Medigap policy, the billing staff will assist you in submitting your claim. If Medicare is your secondary carrier, your claim will not be forwarded to Medicare after your private insurance processes the claim.

NOTE: It is important to be aware that private insurers do not cover all aspects of dermatological services. Patients may be subject to a deductible when seeing providers outside of their network. The definition of a “reasonable and customary” fee by your insurance carrier may be lower than Dr. Prystowsky’s fee schedule. While billing staff will help you with inappropriately rejected claims, no guarantees can be made to the extent of reimbursement that you will receive from a particular insurance company for any given service. To assist you in getting the highest level of reimbursement from your insurance company, it is helpful to bring in your Explanation of Benefits (EOB) for processed claims so that the billing staff can determine if your claim was processed properly for the services provided.

Most patients schedule appointments by calling the office. Every effort will be made to accommodate your request. Telling the receptionist what the purpose of your visit is will help us to ensure that an adequate block of time is scheduled.

Emergency and urgent appointments are made for bleeding wounds that require sutures, skin infections, shingles and ruptured cysts. In cases where an emergency appointment cannot be made, patients are directed to other dermatologists or the emergency room of a hospital.

A concerted effort is made to stay on time with the schedule. Prompt arrival for your scheduled appointment is greatly appreciated. When the office is running behind schedule by more than 30 minutes the receptionist will try to reach you to advise you of this so that you may adjust your schedule accordingly.

For new and existing patients, please take a few minutes to complete our Patient Registration Form prior to your visit. This will help us stay on schedule and reduce unnecessary waiting time for you and your family. For the protection of your sensitive data, we use the highest strength encryption to transmit your information to us.
At Dr. Prystowsky’s office quality care is very important. We strive to accommodate everyone’s schedule, and understand at times conflicts cannot be avoided; however, our office needs sufficient time to fill openings in the schedule to meet patient demands and provide you with the best quality care. When time is blocked out for any appointment, a last minute cancellation means that we cannot fill that time. Last minute cancellations include same day rescheduling, and arriving extremely late to an appointment. Consequently, we require a cancellation fee for last minute cancellations proportional to the time booked for an appointment to cover the facility and labor costs of keeping that reserved block of time for a patient when we are unable to fill vacant spots in our schedule.

In order to assist patients with keeping their appointments and avoiding the need for cancellation fees, it is the office policy to contact the patient prior to the date of the appointment by the phone number provided to the office by the patient. If we cannot reach the patient, we will leave a message on the automated machine or service at the number provided. It is the patient’s responsibility to check these messages and confirm their appointments. If a patient cannot be reached or located, it is still assumed by the office that the patient intends to keep their appointment unless the office is contacted during our normal business hours.

We ask that you provide us with the following notification when canceling an appointment. Regular surgical appointments must be cancelled or rescheduled at least 2 business days before the scheduled appointment. Mohs surgery appointments must be cancelled 5 business days prior to the scheduled appointment. Office visits may be cancelled only if the office is given 24 hours notice during our normal business hours. Only emergency messages are reviewed and forwarded by our service after hours or on the weekends and holidays. Consequently, a cancellation of an appointment after office hours on Friday, on Saturday, or on Sunday will be treated as if it were received on Monday, and cancellations made on holidays will be treated as if it were received on the next normal business day.

If you do not cancel your appointment within the allotted time, the office will assess a fee based on the following fee schedule: $100 for a missed office visit, $500 for an appointment for multiple services requiring us to block more than 1 hour, $500 for a missed surgical appointment, and $1000 for a missed Mohs surgical appointment. If the aforementioned required notification is not provided, we will make every attempt to fill the vacancy and will waive the charge if the spot can be filled. However, there is no guarantee that the spot will be filled on such short notice, and we will require a fee if the spot remains vacant.

Patients with Private Insurance: Dr. Prystowsky does not participate in any insurance plans. Payment for services is expected at the time of visit. You will receive a bill that you may submit to your insurance for reimbursement or request that Dr. Prystowsky’s billing staff submit to your insurance. If the office receives your reimbursement check, Dr. Prystowsky will send you the refund.

An initial office consultation can range from $283 to $740 depending on the nature of the visit and includes a detailed history and physical exam. Any surgical procedures (e.g. biopsies, destructions, or tests) will carry additional charges.
Dr. Janet Hill Prystowsky is a board-certified dermatologist with over 25 years of experience in dermatology and dermatologic surgery. Her mission is to enhance her patients’ wellbeing through the early diagnosis and treatment of skin cancer. Drawing from her training at Columbia University’s Department of Surgery, Dr. Prystowsky has built a thriving practice, removing skin cancer from her patients with a minimally invasive technique called Mohs surgery.

In combination with her focus on early skin cancer detection and removal, Dr. Prystowsky provides a wide range of revitalizing and rejuvenating treatments at her New York City office. She utilizes the latest medical advancements, including laser and tissue-tightening devices, fillers, and botox, to help her patients obtain healthy and beautiful skin. Whether fighting tough rashes or smoothing frown lines, Dr. Prystowsky provides each unique patient a caring and compassionate experience. Under her practiced eye and personal touch, her patients leave confident in their skin after every appointment.

In addition to her private practice, Dr. Prystowsky is a senior attending physician at Mount Sinai Roosevelt/St. Luke’s Medical Center. In 2008 she started her own skin care line as president of LIVAD Skin Care, LLC.

Yes, Dr. Prystowsky sees many children in her practice. She is happy to evaluate childhood skin problems such as warts, eczema, vitiligo, acne, and poison ivy rashes. For very unusual, inherited skin disorders, however, she recommends that you seek a pediatric dermatologist.
