Skin Cancer Surgery
Dr. Prystowsky and her staff perform skin cancer screening and testing. For patients with biopsy proven skin cancer, Dr. Prystowsky performs the surgical removal of the tumor. For melanoma, the tumors are treated with wide excisions. For basal or squamous cell cancers, the tumors are treated by a number of different surgical approaches including Mohs Surgery, when it is appropriate.
Dr. Prystowsky is one of the few doctors trained in Mohs Micrographic Surgery and is a Fellow of the American Society for Mohs surgery.
Mohs surgery is performed in a procedure room under sterile conditions, with local anesthesia. Once the area is numb, the visible portion of the tumor is removed by excision or scraping with a sharp instrument called a curette. Then a thin layer encompassing the under-layer of the tumor is removed. This layer is cut into small sections and a map is drawn to identify the location of each section. Then each piece removed is frozen and the slices are cut, stained and examined under the microscope. Areas where residual tumor is found are marked on the map and these areas are then re-excised. The procedure is repeated until no more tumor is found and the cancer is completely removed. Any bleeding vessels are cauterized or ligated with a suture. Then a pressure dressing is applied.
It takes about 30 minutes for each layer of tissue to be removed and 45 minutes to process and examine it. Most tumors will require one to three layers of removal. Extensive tumors may need additional surgery and require a second visit.
If you have a basal cell carcinoma, there is approximately a 99% chance that you will be cured with Mohs Micrographic Surgery. If you have a squamous cell carcinoma, your chance of cure is 95%. Follow-up visits to detect the rare recurrence are very important. After the initial post-operative period, Dr. Prystowsky will see you, at least annually to monitor the probability of recurrence. If you already have a general dermatologist, you will return to him/her for routine skin care and tumor surveillance. Alternatively, Dr. Prystowsky and her staff will perform these services for you.
Because it is not possible to know the exact size of each skin cancer until it has been completely removed, your post-surgical treatment cannot be entirely determined in advance. Small, superficial wounds can heal on their own with excellent cosmetic results. If a surgical wound requires reconstruction, Dr. Prystowsky will advise you on the benefits of repair by plastic surgery. Dr. Prystowsky utilizes plastic surgery techniques in her repairs. However, for some patients referral to other specialty surgeons may be recommended.
